What is Microdermabrasion?

When does the doctor recommend this treatment?

The doctor often recommends it as part of treatment for active acne. Scaly actinic keratosis skin often requires some form of mechanical exfoliation prior to implementing photodynamic therapy with Metvix or Levulan. Depending on how diffuse the area involved either localized curettage is implemented or generalized microdermabrasion to a diffuse area.

You can think of this as a mechanical exfoliation of only the top 4 microns. When you come to a medical cosmetic facility with multiple platforms you will be advised re the most favorable technology to address your need.

PhotoDynamic Therapy:

Microdermabrasion can proceed with photodynamic therapy with selectively either Amino Levulenic acid 20% Levulan or methylamino levulinate; Metvix depending on the medical condition. We often include it in a package treatment in cases of active acne vulgaris, acne rosacea, actinic keratosis, precancerous skin lesion, sebaceous hyperplasia, and oily skin, and to improve texture and minimize pores. The goal is to incubate the product on the face or other body parts & then activate it with Blu U/ Red light or IPL.  The goal is to open & exfoliate those follicles bearing the sebum. Then we address the bacteria, decrease sebum production & then healing of the acne scars. In the case of actinic Keratosis, we scrape the lesions prior to applying the Metvix. In others we do Microderm abrasion.

Photodynamic therapy (PDT) is especially useful for widespread lesions on the face and scalp. The physician applies a light-sensitizing topical agent to the lesions, then uses a strong light to activate the topical agent, destroying the AKs while sparing healthy tissue. [In Europe, some physicians follow the application of the light-sensitizing agent with exposure to sunlight instead of artificial light; this is known as daylight PDT, and it is considered a gentler treatment than standard PDT.]

Having the light source activate the photosensitizer Levulan or Metvix releases Singlet Oxygen which causes apoptosis or death of rapidly multiplying cells like acne, actinic keratosis, or other precancerous lesions. This yields smoother, healthier skin with improved texture & appearance.

Actinic keratosis often requires maintenance to avoid cancer.

Remember you are under the care of a physician who understands combining standard topical medications with  PDT may also be used alternately every three months, six months, or a year, as determined at follow-up skin examinations.

Remember 10% of actinic Keratosis patients in 10 years will develop skin cancer.

Therefore, PDT is a measure of prevention of skin cancer & another disfiguring scarring.

Occasionally your drug insurance covers the cost of the drug. Therefore the doctor works with her local pharmacy to obtain your insurance approval.

Occasionally we combine the PDT with nonablative fractional therapy for better efficacy.
